Assistance in Coordinating or Managing Life Stages, Transitions and Supports (Coordination of Supports)
Cosmos will coordinate your services with trained, experienced and motivated coordinators who go through regular training. Cosmos coordinators will work with you to assess your needs, manage NDIS services for you, ease difficulties, resolve issues, and make changes to achieve your set goals.
All NDIS participants who have been approved for ‘Coordination of Supports’, are able to access Assistance in Coordinating or Managing Life Stages, Transitions and Supports.
Cosmos will work with you and assist you to:
- Understand NDIS services that you can access through your individual plan
- Assess your needs and set a support plan towards achieving your goals
- Connect you with service providers suitable for your unique needs
- Coordinate services, track your needs and gather reports from health professionals
- Arrange for the Change of Circumstances if needed
- Set a plan towards achieving your goals
- Prepare the final report and submit it with the other reports to NDIS
